WORKPLACE: The employees, walking and working on inclined conveyor belt to unjam and clear the construction debris from the belt were not protected from the hazard of falling between 8 feet 1 inch and 14 feet 1 inch to the floor by the use of appropriate fall protection devices, guardrail systems and/or other equivalent means.

On or about April 21, 2016, an employee sustained severe crushing injury as well as an amputation to the end of the right ring finger and an open compound fracture to the right middle finger while performing the above task using a hand-held hammer.
