29 CFR 1910.26(d): The employer did not ensure that measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semi-trailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard. Location: Loading Dock #3. On or about August 23, 2023, an employee was sweeping loose materials from a dock leveler when the truck assigned to that bay pulled away. The employee fell and was transported to the hospital with lower extremity pain. At the time of inspection, the employer had functional visual warning systems for the dock leveler (red/ green lights) and posted instructions at the location but did not ensure their procedure was adequate to prevent employees from standing on dock levelers or in trailers after they had been authorized to depart. |
