Citation text line 29 CFR 1926.451(b)(5)(i): Each end of a platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end. WORKSITE: The cantilevered ends of the following scaffolds extended more than 12 inches beyond the support and the planks were not secured or installed to prevent accidental displacement: 1. The wood plank roof on the bracket scaffold located on the small porch roof over the front door extended beyond the supports approximately 2 feet on one end and 3 feet on the other end, exposing employees to approximately a 10 feet fall. 2. The wood plank on the roof bracket scaffold located on the section of the main roof at the front of the house extended approximately 2 feet beyond the supports at both ends exposing employees to a fall, exceeding 17 feet. 3. The wood plank on the roof bracket scaffold located on the section of the main roof at the back of the house extended about 2 feet beyond the support at the rack edge exposing employees to a fall, exceeding 17 feet.

WORKSITE: The employees performing roofing work on the following roofs were exposed to fall hazards without adequate fall protection. These include, but are not limited to: 1. The employee, installing blown-in insulation on an unprotected section of the main roof which was a 9/12 pitch was exposed to 17 foot 8 inch fall hazard. 2. The employees installing plywood roof sheathing and roll roofing on the main roof (9/12 pitch) was exposed to 17 foot 8 inch fall hazard. 3. The employees installing roll roofing on the dormer roof (5/12 pitch), was exposed to a 22 feet 8 inch fall. Citation text line 29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. WORKSITE: The employees were accessing the following roof bracket scaffolds using aluminum extension ladders that did not extend at least three (3) feet above the working platform of the scaffold. These include (but were not limited to): 1. The 20 foot aluminum extension ladder used to access the roof bracket scaffold on the porch roof, exposing the employee to a 10 foot fall to the ground. 2. The 32 foot aluminum extension ladder used to access the main roof, exposing the employees to falls in excess of 17 feet to the ground.
