On or about 04/16/2013, the scaffold platform, from which an employee of Sunlight Solar Energy was working collapsed, and the employee sustained serious injuries from the incident. |

Worksite: The wooden bracket of the makeshift carpenters' bracket scaffold platform was only secured to the house wall by using nailsl, and other safe means was not utilized to positively secure the bracket to prevent the bracket from displacement. On or about 04/16/2013, the scaffold platform, from that an employee of Sunlight Solar Energy was working collapsed, and the employee sustained serious injuries from the incident. |
