On or about 9/29/17, the employee standing on the above wooden plank to perform the tasks tripped and fell between 9 feet and 13 feet to the lower level and sustained severe injury to the head and fractured the vertebrae from the incident.

JOBSITE: The employees, wearing tight fitting full face mask respirators while performing spraying insulation activities had not received fit testing prior to initial use of the respirator and at least on an annual basis as required.
