Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) 207 Webster Square Rd., Berlin, CT. Job Site - On or about 11-13-14 - Employee was observed working outside of the warning line system without a fall protection system in use. No personal fall arrest system or safety monitor system was being used. The Roofing Concepts Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 312344260, citation number 1, item number 1 and was affirmed as a final order on 1-26-10, with respect to a workplace located at 1901 Plainfield Pike, Johnston, RI. 02919.
