Employers did not ensure that each employee had access to labels on containers of formaldehyde and to safety data sheets, and was trained in accordance with the requirements of HCS and paragraph (n) of this section: FLAMMABLE SPRAY COATING On, or about July 27, 2016, the employer failed to include formaldehyde in its hazard communication program when refinishers were exposed to formaldehyde while spray coating furniture with Magnalac Lacquer Gloss, Clear Base. The coating contained up to 0.1 parts per million of formaldehyde. Formaldehyde is a skin and respiratory irritant and is a cancer promoter and initiator. |

29 CFR 1910.1052(d)(2): The employer did not perform initial monitoring to determine each employee's exposure to methylene chloride: PANELING On, or about, July 27, 2016, the employer failed to perform initial monitoring to determine each employees' exposure to methylene chloride when employees worked with chemicals including, but not limited to, EverStrong ES130 Contact Adhesive, which contained at least 80% methylene chloride. An employee applying the spray adhesive to office panels was exposed to methylene chloride at an 8-hour time weighted average (TWA) of 0.43 parts per million. The exposure level was obtained from a sample collected over a 154 minute sampling period on August 2, 2016. Zero exposure was assumed for the 326 minutes not sampled. The PEL for methylene chloride is 25 parts per million. |
