29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: Warehouse: On, or about, December 29, 2020, the following walking/working surfaces were in need of repair: A. E-Z Lift Conveyor: The guard rail system installed around the floor opening for the red inclined tire conveyor was loose and pegboard used as an intermediate member was broken. B. Basement stairs near red conveyor: Three stair treads were damaged where the nosing had broken off the concrete surface of each step. C. Lexani Mezzanine: 1. In one area, the plywood floor had split, which created a "soft spot" on the walking surface. 2. A hole was present in the plywood floor where that created a tripping hazard. 3. One of the fixed access ladders was loose. The handrails extended up to the ceiling, but the attachment to the ceiling had partially detached. As employees climbed the ladder, it pulled away from the mezzanine. |
