29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3) of this section) on all open sides: A. On or about August 20, 2015, Insulation Warehouse: The wooden guardrail provided on the storage mezzanine was insufficient in that: 1. The smooth-surfaced top rail was positioned more than 42 inches nominal from the platform surface. 2. The post in the center of the long side of the guardrail was cracked. The rail did not have the strength to withstand the minimum requirement of 200 pounds top rail pressure. 3. When the mezzanine was accessed by powered industrial vehicles, the front portion of the guardrail was removed, leaving an unprotected edge of more than 9 feet, exposing employees to 8 1/2 foot falls. B. On or about August 20, 2015, Garage Door and Gutter Warehouse: 1. The wooden guardrail provided on the storage mezzanine was not provided with an intermediate rail. 2. The front portion of the storage mezzanine, near the ladder access, was not provided with a standard guardrail. |
