The lockout/tagout procedure for the machine did not address cleaning and inspecting the machine for sanitation purposes. The hopper could not be manually cranked during the inspection process and the machine was routinely inspected while running. Additionally, at the time of the accident, a sanitation employee was still in the process of cleaning the front of the machine after the locks had been removed by a Supervisor. Carla's Pasta, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA Inspection Number 1194248, Citation Number 1, Item Number 6 and was affirmed as a Final Order on March 28, 2017, with respect to a workplace located at South Windsor, Connecticut. |

In addition, non-fire resistive hair nets were worn by employees while working on or near live electrical equipment. |
