Location: West Chimney, Yale University Power Plant, 120 Tower Parkway, New Haven, CT 06511 Instance "A": The approximately 7 foot 5 inch diameter platform inside the chimney at the breech are of the chimney was equipped with one 1 1/2 inch thick, 9 5/8 inch wide and 7 foot 1/2 inch long wood scaffold plank and one 1 3/4 inch thick, 9 1/2 inch wide, and 7 foot long wood scaffold plank and therefore was not fully planked. 29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping or have guardrails which block employee access to the cantilevered end.
